Payment:

Due at time of visit by cash or check.  Credit card payment available via PayPal.

Insurance Accepted:

Aetna, Cigna, Empire BlueCross BlueShield, Medicare, and Oxford

A receipt may be provided for you to submit for reimbursement from other insurances/flex spending accounts.

Important Information About Insurance:

Please check with your insurance company to ensure nutrition services are covered and whether or not a referral is needed.  If a referral is required, it must be faxed or mailed to me PRIOR to your appointment.

Copays:  You are responsible for any co-payment your insurance company requires at the time of you visit.  Please refer to the "specialist" copay amount on your insurance card.
