To see if you are covered for visits with me, please call your insurance company (phone number on back of your insurance card) and ask:
- Does my plan cover nutrition counseling/medical nutrition therapy?
- Are there are any limits on the number of visits allowed?
- Is Telehealth covered?
- Do I need a referral or other prior authorization?
- If they ask for certain procedure codes, provide 97802, 97803, 97804, 99404
- Do I have to meet a deductible? If yes, how much?
- What is my co-pay?
- Make sure to note date/time of the call and get the name of the representative you spoke with and a reference number if possible.
Next Step:
If your insurance does cover nutrition services, please call your doctor and ask them to provide any diagnosis code(s) for any of the health conditions for which you are coming to see a dietitian/nutritionist. These codes are necessary to submit claims to your insurance company.

Please note I do not participate with every plan within each of the above insurance companies. For other insurance plans, I can provide a “superbill” which you can submit to your insurance company for possible reimbursement.
